Acute care hospitals

Acute care is a level of health care in which a patient is treated for a brief but severe episode of illness, for conditions that are the result of disease or trauma, and during recovery from surgery.

Long Term Acute Care Hospitals

An LTACH is a long-term acute care hospital that provides medical treatment for patients
requiring continuation of intensive medical care after they have been stabilized in a general
hospital.

Skilled Nursing

A skilled nursing facility is a clinical care provider of 24-hour licensed nursing. A skilled nursing facility is primarily engaged in providing services for residents who require medical and therapy services for the rehabilitation of injured, disabled, or sick persons.

Independent Living

Independent senior living communities are housing designed for seniors 55 and older. Independent senior living communities commonly provide apartments, but some also offer cottages, condominiums, and single-family homes.

Assisted

 Living

An assisted living residence or assisted living facility (also known as retirement home) is a housing facility for people with disabilities or for adults who cannot or chose not to live independently. Assisted Living communities have staff and programs in place that support and assist residents with daily living and basic care in a homelike or apartment setting.

Memory

Care

Memory Care Services are focused on the individual. Dementia slowly interferes with our loved one’s mental ability to manage everyday life. Memory care services create a safe and stimulating environment for those with this type of condition– including its most common form, Alzheimer’s disease.

Physician & Counseling Services

​Physician services refer to the services provided by an individual licensed under state law to practice medicine or osteopathy. It does not include services that are offered by doctors while admitted in the hospital, and charges for which are included in the hospital bill.  Counseling services is a collaborative process that involves the development of a confidential professional relationship that focuses on patients problems.

CARE SERVICES

Home Care is supportive care provided at home. Care may be provided by licensed healthcare professionals who provide medical treatment needs or by professional caregivers who provide daily assistance to ensure the activities of daily living (ADLs) are met.

Hospice SERVICES

Hospice care focuses on the palliation of a chronically ill, terminally ill or seriously ill patient's pain and symptoms by attending to their emotional and spiritual needs.
